Clothing Requirements
Forest Lake requires that each camper has (2) official Forest Lake
"ringer" Tshirts and (2) pairs of matching Forest Lake shorts. These are
important for group pictures and also for uniformity when we take
campers out of camp. You can acquire these items through our official

Activity Consent Form
Meningococcal Meningitis Vaccination
Response Form
Photo Release / Swimming Assessment
Authorization for Non-Prescription
Medications Form
Emergency Medical Card
